By Kevin Hanes, CEO of Reveal Security When I reflect on the years I spent leading one of the world’s largest Security Operations Centers (SOCs) and incident response teams, the lessons learned aren’t just war stories…they’re a playbook for how we should rethink our responsibilities in the face of today’s fast-evolving attack surfaces. Back then, we were managing massive volumes of alerts, hunting nation-state actors, and responding to zero-days on behalf of our clients that “outsourced” their security to us. But the reality is, even with the best people and tools in the world, the hardest lesson always circled back to this: You can’t outsource responsibility. Sure, you can outsource security services. You can hire MSSPs, employ managed detection and response, push workloads to the cloud, and sign SaaS contracts that include the phrase “shared responsibility model.” But at the end of the day, when the breach hits the front page or the regulators come knocking, it’s you. It’s your logo, your customers, your board, and your job. That’s why the SaaS security challenge feels deeply personal to me. I’ve been on the frontlines of the security response when systems get encrypted, stolen credentials are exploited, or anomalous activity gets missed. And I’ve seen what happens when we, as security leaders, let gaps stay open because they’re hard, unfamiliar, or not fully understood. What You’re Not Thinking About Will Hurt You Let me be blunt: most security programs today aren’t truly thinking about their SaaS attack surface. And not because they don’t care, but because it’s sprawling, dynamic, and in many cases, invisible to their current security stack. When you use Salesforce, Workday, Microsoft 365, Google Workspace or hundreds of other SaaS applications, you’re trusting platforms you don’t control, running on infrastructure you don’t own, accessed by users you may barely manage. You might have some visibility at authentication but what about after that?  Your SIEM might capture logs if you integrate it all. But real post-login visibility – actual behavioral understanding of human and non human identities inside the SaaS app? Across multiple SaaS apps?  That’s not something traditional tools were built to handle. Let me ask you this: Can your current tooling detect if a privileged contractor logs in and starts extracting sensitive records from a CRM they rarely use? Can it distinguish a credentialed user doing legitimate business activity in a sanctioned application versus insider abuse? If the answer is “not reliably” or “it would be noisy,” you’re not alone. And that’s the point. The noise is deafening if you try to retrofit old tools into this new SaaS world. You need something purpose-built. Something intelligent. Something that applies ML and AI to understand identity behavior, not just rules looking for known patterns. Something like what we’re building at Reveal Security. Threat Actors Go Where It’s Easy One of the things I used to say to my team was: “Why would a threat actor bother doing anything sophisticated when the basics get the job done and they don’t get caught?” Threat actors are smart. They’ll follow the path of least resistance. Today, that path leads straight into SaaS. Why? Because SaaS is easy. It’s accessible from anywhere. It’s interconnected, with API hooks, third-party integrations, and OAuth tokens galore. It’s complex – meaning it’s hard to monitor and it’s often assumed to be secure because it’s delivered by a big-name vendor. But let’s be honest: your SaaS security probably relies more on trust than verification. And we know how that story ends. We’ve seen nation-state actors using MFA-reset flows in SaaS platforms to impersonate legitimate users. We’ve seen insider threats using SaaS admin privileges to quietly exfiltrate data. And very recently, we’ve seen a major cryptocurrency exchange suffer a data breach driven by the bribery and recruitment of their customer support agents. These “rogue insiders” leveraged their legitimate access to internal SaaS-based customer support systems to steal sensitive customer data.  The harsh truth is, SaaS is the low-hanging fruit of modern cyberattacks. And we, as defenders, haven’t made it nearly hard enough to exploit. Saying The Quiet Thing Out Loud Now, I’m going to say something that doesn’t often get said publicly. I know that feeling – the one where you almost don’t want the alert to fire. Because if it doesn’t fire, you don’t have to know. If you don’t know, you don’t have to respond. And if you don’t respond, maybe no one blames you.  And mostly, they all end up being false positives that just create work. But let me tell you something: that feeling is fleeting. When the breach comes – and it will – everyone knows. Your team knows. Your customers know. Your board knows. And you will carry the weight of the question: Why didn’t we see this?  How did the adversary elude our existing controls? We all got into security because we believe in protecting people, systems, and trust. So let’s not kid ourselves. The goal isn’t to silence alerts, it’s to generate the right ones. The ones that matter. The ones that catch misuse and abuse, even when it’s subtle, after authentication, and inside a sanctioned app. That’s why at Reveal, we don’t just focus on anomaly detection; we focus on behavioral understanding. We track identity journeys inside SaaS apps and cloud infrastructure, building a fingerprint of “typical” and flagging when things deviate in meaningful ways.  Ways that are rare, security relevant and worthy of investigation. Taking Responsibility for SaaS We’re still in the early innings of this game. But here’s the reality: SaaS isn’t going away. CSPM, DSPM, ASPM, SSPM, ESPM — the alphabet soup of Security Posture Management (SPM) tools promises visibility into risk. They map misconfigurations, surface exposure paths and highlight policy gaps. That can be useful. But let’s not confuse awareness with action.

They don’t block threats.
They don’t enforce controls.
They don’t prevent breaches.

SPMs detect, then delegate. A ticket. A Slack alert. An integration call. Protection is someone else’s problem.

To compensate, many posture tools claim to orchestrate security. They integrate with enforcement tools like DLP, CWPP, EDR and WAF. But wiring systems together doesn’t make the system secure.

Coordination ≠ Protection
Visibility ≠ Control
Monitoring ≠ Security So why is there an abundance of SPM vendors?

Because posture is easier.

In healthcare, every minute of downtime isn’t just a technical problem — it’s a patient safety risk.

CNN recently reported that Kettering Health, a major hospital network in Ohio, was hit by a ransomware attack. According to CNN, the Interlock ransomware group claimed responsibility, sending a chilling reminder that healthcare remains a prime target for this particular ransomware gang.

While technical details of the Kettering attack remain scarce, Interlock’s recent history — and the emergence of browser-based attack chains like ClickFix — should put every CISO on high alert. In fact, HHS released a sector-level alert on ClickFix, due to the severity of the attack and how focused attackers were in targeting healthcare organizations. Again, though it is not yet confirmed that this specific exploit was used at Kettering, Interlock has been linked to ClickFix attacks elsewhere.

While Kettering continues to focus on incident response, for those watching across the industry, what’s clear is the growing role of browser-based attacks in ransomware campaigns, and how, in healthcare environments, these kinds of attacks can be life-threatening.

Healthcare Remains a Top Target

Ransomware actors continue to prioritize healthcare organizations as targets because of the high value placed on continuity of care. Timely access to records, imaging, and communication systems is essential for delivering treatment — so when those systems are disrupted, the operational impact is immediate.

Healthcare organizations are uniquely vulnerable to ransomware for several reasons:

• Dispersed and unmanaged endpoints: Clinicians often use shared workstations, personal laptops, and mobile devices, increasing attack surface.
• High uptime requirements: Any downtime can delay surgeries, diagnostics, or emergency care — forcing quick, sometimes quiet, ransom payments.

Understanding ClickFix and Browser-Based Tactics

ClickFix is a technique that relies on seemingly legitimate browser interactions — like fake CAPTCHAs or pop-up prompts — to trick users into activating malicious scripts. What makes this approach difficult to detect is that it relies heavily on the browser for clipboard manipulation, with no file download to trigger traditional file-based alerts or network signatures.

Here’s how a ClickFix attack could unfold in a healthcare organization:

1. A healthcare worker visits a compromised website or clicks a phishing link.
2. A fake browser prompt appears (e.g., “Please complete this CAPTCHA to continue”).
3. The user is tricked into copying and running a script (often a PowerShell command), which silently installs malware — sometimes without any download or obvious warning.

SquareX first detailed this technique in 2024 to illustrate a broader challenge: many endpoint and network tools lack visibility into what happens in the browser — including browser extensions, clipboard activity, and in-browser scripting.

Why Traditional Security Tools Miss Browser Attacks

Most healthcare organizations rely on a familiar stack: Secure Web Gateways (SWG), Endpoint Detection & Response (EDR), and Data Loss Prevention (DLP). But these tools were not designed for the complexity of modern browsers.

What they miss:

• DOM-level changes (malicious form injections or script manipulations)
• Malicious browser extensions (often installed by non-technical staff)
• Clipboard hijacking (stealing credentials or session tokens)
• Client-side file and data reassembly (a malicious file that is broken into parts to avoid detection and after all pieces are downloaded, it is reassembled into a full payload)
• WASM-based payloads (running near-native code inside the browser)

Meanwhile, many clinical workflows — from EHRs to claims portals — now run in the browser. In fact, a 2024 survey of the healthcare technology market showed healthcare SaaS expecting to grow nearly 20% by 2028.

Do we have the visibility we need at the browser layer?

Are we prepared to detect and respond to client-side threats that bypass legacy controls?

In today’s competitive landscape it has become an increasingly important for businesses looking for ways to adapt their data security, governance, and risk management practices to the volatile economy by improving efficiency or reducing costs while maintaining structure, consistency, and guidance needed to manage cyber risks, and ensure compliance.

Application Workload and Sensitive Data on the Move

As organizations increasingly migrate various on-premises applications and data workloads to multicloud environments, the complex and dispersed nature of cloud environments creates significant challenges arising from managing vulnerabilities, controlling access, understanding risks, and protecting sensitive data.

What is Data Security Risk?

Data security is the process of protecting digital information from unauthorized access, corruption, or theft throughout its entire lifecycle. Risks are introduced to the databases, file servers, data lakes, cloud repositories, and storage devices through all the access paths to and from these systems. Most important the data itself, in motion and at rest, deserves the same attention to protection. When properly implemented, a data-centric strategy will protect an organization’s assets and data against cyberattacks, as well as guard against insider threats and human error, which remains among the leading causes of data breaches.

Why does complexity factor into data security risk?

Many factors drive organization’s growth and at the same time security complexity. Complexity inhibits operational stability, and equally impacts security stability. In a recent study conducted by Cloud Security Alliance, Understanding Data Security Risk 2025 Survey Report , CSA highlights key areas that organizations are currently addressing:

• High growth with AI-driven innovations and security: As AI accelerates innovation, it also amplifying the threat landscape. Rapid expansion often outpaces the development of needed infrastructures, processes and procedures, leading to ad-hoc measures that increase complexity. Gen-AI also adds a new layer of complexity as it becomes more prevalent in cloud environments that continue to be a prime target due to their complexity and scale.

• Processes and automation:We know that limited staff and inefficient or outdated processes often lead to manual and redundant efforts. This becomes a huge burden on teams who struggle to keep pace resulting in reactive stopgap or workaround actions. All of this to say, manual efforts can be error-prone and time-consuming. At the same time, organizations may experience unnecessary bottlenecks that can exacerbate complexity and interfere with risk identification and security enforcement. Automate as much as possible including data security and risk intelligence to ensure that risks are proactively mitigated therefore reducing escalation of high-urgency incidents. 
  Only 8% reported that their organization is fully automated, while 54% use semi-automated processes; and 22% rely on manual methods.

• Standardization: The absence of prescribed security policies, standards, and security tools utilized across processes and systems can lead to inconsistent practices, making governance, security, and posture management execution riskier and more complex. This leads to inefficiencies and conflicting information. Who discerns which alert to prioritize and what action is escalated? It shouldn’t be a judgement call rather an informed investigation based on ongoing contextual integrated insights and severity-level assessment.
  Over half of organizations cited they have the tools to identify the number of data sources that are the riskiest but they on average deploy four or more tools to assess and manage risks.

• Regulation and compliance: The need to comply with an ever-increasing array of regulations adds layers of complexity in the form of compliance responsibilities, reporting requirements, and controls. A heavy focus on compliance can leave organizations unprepared for evolving attack surfaces and sophisticated cyber threats. While compliance frameworks like ISO, GDPR, and PCI DSS are essential for establishing baseline security measures, they often emphasize reactive responses and may not cover all emerging threats. For modern data security, organizations need to shift toward a more proactive, risk-based approach featuring continuous compliance monitoring and reporting to meet audit requirements and avoid costly penalties.
• Technology integration: While technology offers solutions for effectiveness and efficiency, the integration of multiple systems without strategic oversight can lead to inefficient security infrastructure, a mismatch of security stack components, creating disconnected silos of security processes. The hidden costs of disjointed tools are fragmented visibility, control, and access enforcement. Traditional compliance and security tools, while essential, often lack the scalability and integration needed for modern and effective risk management.

• Confidence levels: In a multicloud world, blind spots breed vulnerabilities. We are seeing many organizations lack confidence in identifying high-risk data, even when they have tools in place. Often overlooked are functions that live in different silos such as monitoring, discovery, threat detection, encryption, and access enforcement. This separation or divided approach inevitably becomes hard to maintain a consistent security posture.

  The report reveals that management's strategic priorities aren’t compatible with staff's operational realities. For example, 10% of staff reported being “not at all confident” in identifying high-risk data sources, compared to 3% of management.
